Homecare Clinical Nurse Advisor – TPN (Outpatient Services)

Location: Wigan / Warrington

Contract: Permanent, full time

Hours: 40 hours per week

We are seeking an experienced and compassionate Clinical Nurse Advisor to support patients receiving Total Parenteral Nutrition (TPN) through a specialist outpatient homecare service.

This role focuses on delivering high-quality clinical care, education, and ongoing support to patients living with complex nutritional needs, enabling them to safely and confidently manage TPN therapy in their own homes and community settings.

The role

Working across a specialist outpatient programme, you will support patients from initiation through to long-term management of TPN therapy. You will provide hands-on clinical care, structured education, and ongoing follow-up, while working closely with multidisciplinary teams to ensure safe, effective, and patient-centred delivery.

You will be trusted to manage your own caseload and contribute to service quality through audit activity, service review, and continuous improvement.


Key responsibilities

  • Clinical care & patient education
  • Deliver high-quality clinical care in line with agreed Service Operating Procedures
  • Provide structured education and ongoing support to patients, families, and carers receiving TPN therapy
  • Support patients within their own homes and approved community settings
  • Assist with therapy initiation, follow-up visits, and ongoing assessment
  • Promote patient confidence, independence, and adherence to therapy
  • Maintain and manage all clinical equipment and materials
  • Collaboration & service delivery
  • Build strong working relationships with dietitians, nutrition nurses, pharmacists, and wider healthcare teams
  • Support outpatient pathways and continuity of care between hospital and home
  • Deliver services in line with agreed KPIs and service expectations
  • Identify, manage, and escalate adverse events in line with policy
  • Complete all clinical documentation, activity reporting, and administration accurately and on time
  • Quality, safety & governance
  • Maintain high standards of documentation and clinical governance
  • Complete all mandatory and programme-specific training
  • Adhere to safeguarding, pharmacovigilance, health & safety, and regulatory requirements
  • Maintain professional registration and ongoing CPD (including NMC/NMBI revalidation)
  • Work in line with professional codes of practice


What we’re looking for

Essential

  • Registered Nurse with current NMC or NMBI registration
  • Minimum 1 year post-registration nursing experience
  • Full UK or Irish driving licence
  • Excellent communication skills and the ability to build trust with patients and families
  • Strong organisational skills with the ability to manage a caseload independently
  • A patient-centred approach and interest in long-term condition management
  • Confident using digital systems and Microsoft Office applications
  • Satisfactory enhanced DBS (or equivalent) and pre-employment medical checks


Desirable

  • Experience in nutrition support, gastroenterology, intestinal failure, oncology, or IV therapies
  • Previous exposure to TPN, enteral feeding, or complex homecare therapies
  • Degree-level education or post-registration qualification in a relevant area
